Hi!
I haven't been here long, but the way I see it, you rate whenever you reach a decision about the site. I don't go into every nook and cranny looking for a whiff of malware, I just take into account previous experiences, other users' ratings, and the reputation of that site, as well as some other things.It shouldn't be a complicated matter, and you can always change your rating.

Check the background...
You could check the background of the site. On WOT Security Scorecard click "whois": here you can see background data about the site. And then you could ask:
Is the site trustworthy if it claims to be 10 years old, but whois reveals that it started only yesterday..?
Is the site trustworthy if it claims to be open and transparent, yet hides itself behind total anonymity..?
Is the site trustworthy if it claims to be american, yet it's owned by a russian and hosted in Ukraine (or reverse)..?
Is the site trustworthy if it claims to be uniquie, yet whois reveals that the owner also owns 2000 other domains (all with identical content)..?
Is the site trustworthy if it claims to be widely known, and a global leader in its field, but no search engine has ever heard of it..?
And so on...

When and how to rate
I have not found answer to the most basic question: when and how to (which mark) rate?
For example, I regularly open
http://ereptools.net/currencies/local
The web page always behaves the same. Simply opens, nothing else happens. I do not open other pages from this site (except 1 or 2).
If, for example, I loaded the page 100 times (during for example 100 days), and the page did not harm me (at least I do not perceive anything) (does not change at all), then it is time to rate it? "Excellent"? Or "good"? Or, a bit sooner than 100 days?
What investigations should I do?
What (anti-malware) software should I install?
And an important thing: I use Firefox, and extensions which block ads and javascript. So let's suppose the page is nasty, but nastyness is in content parts which are blocked (not shown) at me. I could (falsely) believe that it is an innocuous page..
When and how rate others?
(And, consequently, how can I trust, what should I think about the page?..)
