If WOT blocks a site does malware get stopped from downloading???

If you have configured the add-on to actually block access to the site, then yes, bad sites won't have a chance to start malware downloads. However, if the add-on is configured to just warn you, it's up to you to not fall for any download requests.

You can enable blocking for each rating component from the Warnings page of the add-on settings.
