This site has warning from QUttera "malicious blacklisted domain" but I think this reflects the nature of a site free to use to create web forms. In one file sample of 35 they found 2 malicious ( I think 4 Nov 2016) but sample on 6 Nov was just 1 suspicious. So be cautious.

This is a form builder, a tool for webmasters and anybody can use this tool for free or through a paid subscription and start building forms. There are hundreds of users that come to the site and build forms with phishing purposes (login forms of popular sites, etc.), but they are usually detected by their phishing detection tool, some of them bypass the filter though. So, people should not rate bad this site just because they see the domain in the link to the form, this is just the domain where the form is hosted. The company itself provides great tools to make outstanding forms. They even have a Templates Gallery and a Theme Store where hundreds of ready to use forms are there for free, anybody can grab and customize them. That's why I trust 100% this site
